Thin-section CT and histologic findings are correlated in various types of uncommon pneumoconioses associated with arc welding, graphite, aluminum, and hard metals. Although the CT findings of the pneumoconioses in this retrospective study were variable and nonspecific, the pathologic findings for each type of pneumoconiosis were characteristic. Thin-section CT may prove to be of value in assessing pneumoconioses in larger prospective studies.

Mineral analyses of lung tissues for certain types of inhaled particles provide an accurate reflection of exposure, particularly when combined with epidemiologic information. Although technologic advancements in this area have been impressive, the review appropriately describes important indications, procedures, and limitations of mineral analyses. Asbestos-related lung diseases, silicate pneumoconioses, and metal-induced diseases are discussed.

Am Ind Hyg Assoc J 1995, 56:490-498. The lungs of 15 former workers exposed to silicon carbide were evaluated with analytic transmission electron microscopy and energy dispersive spectroscopy. Exposure ranged from 23 to 32 years, and exposure cessation ranged from 5 to 9 years. Silicon carbide fibers of 5 μm or longer, angular silicon-containing particles, and ferruginous bodies were found in higher lung concentrations in workers with lung fibrosis, with and without lung cancer, than in workers with neither lung disease. This is the first reported human series to estimate the persistence (durability) of silicon carbide particles in the lungs.

Titanium exposure is generally considered to be benign. This case report documents pulmonary alveolar proteinosis associated with chronic titanium exposure. The report is unique and supports titanium exposure as another potential cause for this condition. This observation is another example of a supposedly inert and benign inhaled agent overwhelming lung defense mechanisms to produce clinical lung disease.

Although PVC is most commonly associated with asthma in the work place, this case report demonstrates that inhaled PVC dust can also elicit ILD and related immunologic changes that may result in extrapulmonary manifestations such as systemic sclerosis. The patient had two rare diseases that may be considered independent events. However, the simultaneous development of pneumoconiosis and systemic sclerosis and other information support the conclusion that both occurred as a result of PVC exposure.
