Using the American Cancer Society estimates of incidence and deaths for lung cancer along with the odds ratio for survival from the Mayo Lung Project Study (2.2), they estimate that 315,540 additional high-risk patients might have survived between 1980 and 1994 had annual chest radiographic screening been done.

Ann Thorac Surg 1995, 60:1573-1582. A prospective nonrandomized study of 99 patients evaluating the use of PET scanning for preoperative staging. Seventy-six patients underwent PET and CT scans of the mediastinum as well as surgical staging. The positive and negative predictive values for mediastinal disease were 88% and 92%, respectively, for PET and 54% and 79%, respectively, for CT scanning. The accuracy of PET scanning was 91% whereas that of CT scanning was 70%. Using whole-body scanning, 11 patients (11%) were shown to have previously undetected metastatic disease; there were no false positives.

    • The Canadian Lung Oncology Group: Investigation for mediastinal disease in patients with apparently operable lung cancer. Ann Thorac Surg 1995, 60:1382-1389. This multi-institutional study recruited 685 patients with operable NSCLC. Patients were randomly assigned to undergo either CT scanning or mediastinoscopy. If either study was negative the patient proceeded to thoracotomy. If the mediastinoscopy demonstrated tumor, the disease was considered unresectable. If the CT scan was positive, the patient then underwent mediastinoscopy. If these findings were also negative, the patient underwent thoracotomy. The endpoint of the study was thoracotomy without cure, ie, unresectable disease, incompletely resected disease, or recurrent tumor. The patients in the CT group were 5% less likely to undergo thoracotomy without a cure. The investigators found no benefit for mediastinoscopy without CT scan. They recommend mediastinoscopy for all patients with nodes greater than 1 cm.

    • Randomized trial of lobectomy versus limited resection for T1 N0 non-small cell lung cancer
    • Lung Cancer Study Group: Randomized trial of lobectomy versus limited resection for T1 N0 non-small cell lung cancer. Ann Thorac Surg 1995, 60:615-623. Of 771 registered participants, 276 were found to be eligible for randomization and of these 247 were randomly assigned to either lobectomy or limited resection (125 and 122 patients, respectively) via thoracotomy. Six patients in the lobectomy group required mechanical ventilation for more than 24 hours postoperatively; otherwise, there was no difference between the two groups in terms of mortality and morbidity. When survival differences were considered in all 276 patients, there was no difference between the groups; however, examining the data from only the 247 there was a slightly lower death rate and death with cancer rate in the lobectomy group. There was a tripling of locoregional recurrence in the limited-resection group. The investigators did not detect any difference in late pulmonary function between the two groups.

J Thorac Cardiovasc Surg 1995, 110:386-395. This study prospectively evaluated 126 consecutive patients who underwent resections for locally advanced cancers (T3 or T4). Although patients undergoing extended resections had an increase in morbidity compared with those who did not, there was no difference between the groups in terms of mortality, severe complications, or multiple complications. Patients underwent surgery regardless of a suspicion of mediastinal organ involvement. The only patients who died (7.5%) were those who had a pneumonectomy (53%). The authors justify this approach by noting the increased survival in those who had complete resections. They did not find age to be a contraindication to an extended resection. There was no difference in survival between patients with T3 or T4 tumors or between N0 or N1 versus N2 nodal status. The best survival was in patients undergoing complete resections.

J Thorac Cardiovasc Surg 1995, 109:473-485. A multi-institutional phase II trial evaluating neoadjuvant chemotherapy in patients with stage IIIA disease proven by mediastinoscopy. Patients received two cycles of cisplatin and vinblastine. Patients whose disease was stable or who responded underwent thoracotomy (88%). Of the patients who underwent exploration, 75% had what appeared to be resectable lesions, but in fact only half of these were completely resectable. The patients then underwent two more cycles of chemotherapy followed by radiation. Only 45% of the patients completed the protocol. For patients who had a complete resection, there was a significant survival benefit.

This is another small phase II trial with stage IIIA patients who received preoperative chemotherapy consisting of platinum, 5-fluorouracil, and vinblastine along with accelerated radiotherapy. This regimen was followed on treatment day 57 by resection. Postoperatively an additional cycle of chemotherapy and radiation was given. There was one preoperative death and two operative deaths; thus three of 40 died from the treatment. Downstaging was seen in 46%, with 14 patients total having sterilization of their N2 disease. The patients whose disease was downstaged had a significant survival benefit.

Twenty-two patients with stages IIIA and IIIB lung cancers underwent accelerated induction therapy with cisplatin, 5-fluorouracil, and etoposide, concurrent with a fractionated radiation dose of 27 Gy. The resection rate 4 weeks later at surgery was 79%. Postoperatively the patients received a second course of chemotherapy and radiotherapy (13 to 36 Gy). There was one induction and four postoperative deaths. Despite the toxicity, there was a 40% rate of downstaging, along with excellent local control. It remains to be seen whether there is an increase in survival.
