High alcohol consumption (over 80 g/d for women and over 100 g/d for men 2 years prior to admission) was identified as a major risk factor for developing CAP and conferred a worse prognosis in this case-control study. Other studies have identified the negative aspects of excessive alcohol intake, but the case-control construct of this study isolated the effect of alcohol from comorbid factors such as malnutrition, cirrhosis, chronic obstructive pulmonary disease, smoking, age, diabetes mellitus, and heart failure.

These investigators demonstrate that specific P. aeruginosa adhesins recognize a target compound, asialo GM1, overexpressed in CF epithelial cells. Binding of this receptor incites interleukin 8 release. Furthermore, a P. aeruginosa-derived signaling compound, autoinducer, also causes interleukin B release. This study represents the latest installment by a group of investigators who have made significant contributions toward understanding the molecular cross-talk between P. aeruginosa and CF bronchial epithelium.

This in vitro study demonstrates that CF bronchial epithelial cells are undersialylated compared with the same cells rescued by transfection with DNA-encoding cystic fibrosis transmembrane conductance regulator. Undersialylation results in increased numbers of asialo GM1 on the cell surface, a target for P. aeruginosa and S. aureus adherence. This establishes a plausible link between the genetic defect in CF and chronic infection.

Pneumoniae interactions with host cells. They describe these recent developments in terms of how they relate to the generation of the classic symptoms and signs of pneumococcal pneumonia. The result is an excellent perspective of the pathogenesis of pneumococcal pneumonia and what the research data indicate for future therapeutic approaches.

This study demonstrates that a ligand in the cell wall of S. pneumoniae binds specifically to PAF-R, which is probably the most important step toward the development of invasive disease at the cellular level. Furthermore, these investigators show that blocking this step can reduce the incidence of bacteremia in an animal model. Such work illustrates again that bacterial address-recognition on the surface of epithelial cells is not an isolated, benign event, but in fact can be the initiation event of a well-developed mechanism to subvert the host's natural defense system and set off an array of bacteria-host responses.

In contrast, this study examines in detail the cellular consequences of P. aeruginosa attachment. The authors show that P. aeruginosa attachment leads to a specific pattern of destruction of the barrier function of the epithelium.
