This article contains data on the serotype distribution of all S. pneumoniae isolates from blood or cerebrospinal fluid submitted to the Centers for Disease Control and Prevention in whom the vaccine status of the patient was known. Pneumococcal vaccine efficacy was calculated from a formula that is based on the assumption that vaccination has no effect on the incidence of nonvaccine-type infection. The large number of isolates allows vaccine efficacy estimates to be made for individual serotypes of S. pneumoniae and for subsets of patients.

In this report of a case-control study of pneumonia and influenza that was performed at the Kaiser Permanente Northwest Region health maintenance organization, non-use of influenza vaccine in high-risk patients was associated with an increased risk of hospitalization for influenza and pneumonia and increased risk of hospitalized death. The savings for the health maintenance organization were $6.11 for high-risk, elderly patients. Conversely, the health maintenance organization incurred a net cost of $4.82 for each vaccination of a non-high-risk elderly person.

The efficacy of influenza vaccination was studied in patients over 64 years of age who were enrolled in Group Health, Inc., a health maintenance organization in Minneapolis-St. Paul, over three consecutive "flu seasons." Vaccine recipients were generally sicker at baseline. However, during each year there was a significantly reduced risk of hospitalization for pneumonia and influenza, of hospitalization for all acute and chronic respiratory illnesses, and of mortality from all causes among the influenza vaccine recipients. Direct savings averaged $117 per year per person vaccinated.

Young working adults (18 to 64 years of age) were enrolled in a double-blind, placebo-controlled study of influenza vaccine efficacy. The primary outcomes were upper respiratory illness, absenteeism from work because of upper respiratory illness, and visits to physicians because of upper respiratory illness. The authors noted significantly fewer upper respiratory illnesses, fewer days of sick leave because of upper respiratory illness, and fewer physician visits because of upper respiratory illness in the vaccinated subjects. The cost savings were estimated to be $46.85 per person vaccinated.
