The authors found that advanced age (70 years or older), Simplified Acute Physiology Score higher than 12, presence of septic shock, need for mechanical ventilation, bilateral pulmonary involvement, and infection caused by P. aeruginosa were all associated with fatal outcome from severe CAP.

Retrospective chart review of 28 patients with alcohol abuse admitted with CAP. The authors found an overall mortality rate of 64.3%. In those patients with bacteremic K. pneumoniae (39.3%), the mortality rate was 100%. All patients with bacteremic K. pneumoniae required ICU care and mechanical ventilation, and all isolates were susceptible to the antibiotics used.

The authors found that mortality was associated with age older than 60 years, septic shock, impaired alertness, mechanical ventilation requirement, bacteremic pneumonia, and S. pneumoniae or Enterobacteriaceae as the cause of pneumonia. This study found no mortality benefit in knowing the etiologic agent causing pneumonia.

Invasive disease secondary to intermediate-resistant S. pneumoniae is associated with immunosuppressive disease, previous β-lactam antibiotic use, previous hospitalization, and hospital-acquired infection. Invasive disease secondary to highly resistant S. pneumoniae was associated with previous β-lactam antibiotic use.

The study found that in patients without underlying risk factors (HIV, steroid dependence, malignancies, sickle cell disease, and nursing home residence), the yield of routine blood cultures is minimal (6.6%), and blood cultures led to antibiotic change in only 1.4% of the patients studied.
