The authors describe an observational study of 471 domiciliary patients, concluding that the Winnipeg criteria ( Anthonisen et al. Ann Intern Med 1987, 106:196-204) only define an exacerbation. Clinical criteria did not predict severity, but a return to the practitioner with unresolved symptoms (clinical failure) was predicted by a history of frequent recent exacerbations and coexistent cardiopulmonary morbidity.
