Results suggest that Peromyscus mice, particularly P. maniculatus, may be the main reservoir of the virus in California, confirming previous data from New Mexico and Arizona.

The authors compared 17 case-patients with three control groups (case-patients households, near- and farhouseholds). Significant risk factors were peridomestic cleaning, agricultural activities, and an increased number of small rodents in the household.

Clinical and laboratory findings, diagnostic techniques together with autopsy pathologic data are described thoroughly. Furthermore, intriguing inferences are drawn between pathology and clinical course, providing a clue to the definition of HPS as a pulmonary capillary leak syndrome.

Hum Pathol 1995, 26:110-120. This interesting article represents an important step in the understanding of HPS pathogenesis from a pathological point of view. Antemortem and postmortem pathological findings were evaluated in nine surviving and 13 dead patients. The authors confirm the lung as the primary target of the virus (capillary leak syndrome restricted to the pulmonary circulation) and HPS as a unique constellation of pulmonary, hematologic, and reticuloendotelial pathological abnormalities.

Excellent review of radiographic findings in HPS. The authors provide additional understanding to the pathogenesis of this atypical adult respiratory distress syndrome (mainly interstitial, with possible evolution to alveolar flooding with nonperipheral pattern). They further discuss the link among radiographic, clinical, and pathological findings, suggesting that the recognition of this unique radiographic pattern might be important in the differential diagnosis of adult respiratory distress syndrome.

An excellent study on in vitro effects of chlamydial infection on ciliary motility of bronchial epithelial cells. The authors characterized the inhibitory effect of C. pneumoniae on the ciliary activity. They demonstrated that C. pneumoniae inhibition of the beating of ciliated bronchial epithelial cells was probably caused by the structural moiety of C. pneumoniae particles that exert a toxic effect. The results support the hypothesis that decrease of ciliary motility might be an important facilitating factor in C. pneumoniae respiratory infections. Moreover, toxic effects to epithelial cells may account for some of the clinical manifestation of acute C. pneumoniae infection or chronic infection, such as development of chronic bronchitis.

N Engl J Med 1995, 333:474-480. An excellent 10-year prospective study on the effects of S. pneumonias drug resistance on mortality. Among 504 patients, the authors found 29% and 6% penicillin- and cephalosporin-resistant strains, respectively. Interestingly, after adjusting for other predictors of mortality, the odds ratio for mortality in patients with penicillin-resistant strains was 1. Furthermore, in patients with penicillin and cephalosporin-resistant strains treated with penicillin and cephalosporin the mortality was not significantly different in comparison with patients with sensitive strains. The authors conclude that penicillins and cephalosporins remain the treatment of choice for severe pneumococcal pneumonia.

An extremely detailed review on Streptococcus pneumoniae antibiotic resistance. Interestingly, the authors report an increasing rate of erythromycin resistance (6% in United States during 1992) and the appearance of multiple drug-resistant strains, occurring in only four capsular types (6B, 14, 19A, and 23F). Risk factors seem to be advanced age, prior use of antibiotics, and hospitalization. Finally, they emphasize the need to develop a better vaccine for the primary prevention of pneumococal disease.

After an excellent descriptive narrative on pneumococcal resistance history, the author discusses its genetic mechanisms and geographic spread. Therapeutic management is illustrated, in which high-dose penicillin (1,200,000 U/2 hours), ceftriaxone, or vancomycin are suggested, remembering the penicillin's inability to reach adequate levels in the subarachnoid space. Interestingly, the author suggests to immunize all adults, thereby reducing the incidence of potentially fatal infections and the pneumococcal carrier rate in the general population.
