This is one of the largest studies examining the acute incidence of akathisia in a recently admitted inpatient population. In the first 2 weeks, 41% of 100 consecutively admitted patients developed mild akathisia, and an additional 21% developed akathisia in the moderate to severe range.

Halstead SM, Barnes TRE, Speller JC: Akathisia: prevalence and associated dysphoria in an in-patient population with chronic schizophrenia. Br J Psychiatry 1994, 164:177-183. In this study, 120 chronic inpatients were examined for the prevalence of different subtypes of akathisia. Chronic akathisia was diagnosed in 24%, and pseudoakathisia, in which there is no subjective complaint of restlessness, was diagnosed in 18%. Younger patients seemed at greater risk for akathisia and men were at greater risk for pseudoakathisia.

Fenton WS, Wyatt RJ, McGlashan TH: Risk factors for spontaneous dyskinesia in schizophrenia. Arch Gen Psychiatry 1994, 51:643-650. The authors performed a retrospective record survey on 100 patients in the Chestnut Lodge follow-up study of patients who had never been treated with neuroleptic medication. They found evidence of a movement disorder in 23%, with 15% have clear-cut documentation of orofacial dyskinesias. These numbers are higher than most previous reports of spontaneous dyskinesias, which have generally been in the range of 5-10%.

Arch Gen Psychiatry 1995, 52:478-486. This is a large-scale study of risk factors for tardive dyskinesia in a group of patients aged 60-100 years, with a mean age of 75 years. Whereas only 16% of patients with a duration of neuroleptic treatment of less than 3 months had tardive dyskinesia, this increased to 29% for 3-12 months, 30% for 1-10 years, and 41% for greater than 10 years.

This is an interesting report which points out that tardive dyskinesia may occur in a nonpsychiatric setting, specifically as a result of the antiemetic neuroleptic drug metaclopramide, which is frequently used for diabetic gastroparesis and for nausea resulting from chemotherapy.

The investigators performed a positron emission tomography study of regional cerebral blood flow in 10 patients with Parkinson's disease and major depression and compared the findings with those of 10 patients with Parkinson's disease without depression, 10 patients with primary depression and 10 control individuals. In both the depressed Parkinson's disease group, bilateral decreases in blood flow were observed in the medial prefrontal cortex and cingulate cortex. Similar findings were seen in the medial prefrontal cortex of the primary depression group, but not of the other groups. These findings indicate that the medial prefrontal cortex may be involved in depression.
