J Antimicrob Chemother 1994, 34:545-553. This paper describes the enhancement of microbicidal activity of neutrophils by rufloxacin. The authors demonstrated this augmentation of neutrophil killing of Klebsiella pneumoniae using both in-vitro and ex-vivo techniques. In addition, in a mouse model of Candida albicans sepsis, rufloxacin markedly improved survival from 5.5% in the placebo group to 87.5% in the treated group.

These authors demonstrated that neutrophils from patients with infection demonstrated marked increases in the phagocytosis of fluorescein-labelled bacteria after they were treated with either cefodizime or ceftriaxone. These studies also demonstrated that post-treatment neutrophils generated higher quantities of reactive oxygen intermediates.

Infect Immun 1995, 63:2236-2242. Log phase E. coli were incubated with concentrations of ceftazadime, ciprofloxacin, gentamicin, or imipenem in whole blood preparations. Lower quantities of cytokines were produced in preparations containing gentamicin or imipenem than in those containing the other two agents. The mechanism was probably related to greater quantities of LPS liberated in E. coli incubated with ceftazadime. Gentamicin may have released less LPS and the authors also suggest that binding of LPS by gentamicin may have further reduced the bioavailability of LPS.

Clindamycin suppressed TNF-α production by human peripheral blood mononuclear cells stimulated by LPS by approximately 40%. This may partly explain the greater efficacy of clindamycin compared with penicillin in the treatment of fulminant group A streptococcal infections such as necrotizing fasciitis and streptococcal toxic shock syndrome.
