Deakin JFW: Neuropsychological implications of brain changes in schizophrenia: an overview. Psychopathology 1994, 27:251-254. This theoretical overview provides a good summary of the 'basolateral circuit' hypothesis. It follows previous demonstrations by the author that there are increases in radioligand binding to the glutamate uptake site bilaterally in the ventral frontal cortex. The author suggests that this circuit, composed of ventral frontal cortex, anterior temporal cortex and amygdala, and the dorsomedial nerve of the thalamus, may play a critical role in social communication deficits and symptom formation in schizophrenia. The previously cited references [7,8] are empirical studies that stem from this theoretical position.

Magnetic resonance imaging and neuropsychological tests were admistered to 68 patients with schizophrenia and 68 healthy control individuals, making this one of the largest and methodologically most sophisticated studies of structure-function relations in schizophrenia. The patients had smaller brain volumes, and brain volume correlated with cognitive performance, but cerebrospinal fluid volumes and ratios had little relation to cognitive measures.

Data were pooled from four independent cohorts of patients with schizophrenia, with sample sizes ranging from 20 up to 128; in nearly 100 comparisons between men and women there were very few differences, and none of these suggested greater impairment among men. The authors acknowledge as a limitation that the results should be considered relevant only for chronic patients with onset of schizophrenia before the age of 30 years.

The mesiotemporal lobe was measured on magnetic resonance images, and divided into three compartments (amygdala, anterior hippocampal formation, and posterior hippocampal formation) in 29 first-episode patients. Only the anterior region correlated with neuropsychological function. There was no significant correlation with memory functions, as may be expected based on lesion studies, but there were significant correlations with executive and motor functions that are usually attributed to the frontal lobes.

Am J Psychiatry 1994, 151:1437-1447. The major cortical regions and asymmetries were measured on contiguous 3.1 mm thick magnetic resonance images in 70 patients and 51 control individuals. Cortical asymmetries in control individuals were consistent with predictions based on previous literature examining only linear or area measures, and asymmetries were more prominent in dextrals and men. Patients had normal cortical volumes but lacked the normal asymmetries. Lack of asymmetry was associated with nonparanoid diagnosis.

A methodologically sophisticated study involving reconstruction of superior temporal plane surfaces, showing expected left to right asymmetries in 12 out of 14 healthy dextral control individuals, and reversal of asymmetry in 13 out of 14 dexlral patients. There was no difference between groups in Heschl's gyrus. Severity of thought disorder was related to asymmetry (patients with most marked reversals had least severe thought disorder).

Arch Gen Psychiatry 1995, 52: 657-667. One of the largest controlled studies of glucose metabolism (42 patients and 42 control individuals), coregistered with anatomic magnetic resonance images. There were no resting metabolic abnormalities in any brain region, but abnormal gradients were found, particularly higher left-right gradients, in both first-episode and previously treated patients. Greater relative activity in left lateral dorsal prefrontal and midtemporal regions was associated with more severe hallucinations.

The author focuses on the possible impairments in 'connectivity' between frontal and temporal regions, which in healthy individuals show highly correlated patterns of activation, which do not appear in schizophrenia.

Six patients who tended to hallucinate consistently were compared with six who tended not to hallucinate. Patients engaged in reading (a control task), an 'inner speech' task (reciting sentences formed in patients usual 'inner voice'), or an 'auditory verbal imagery' task (imagining sentences spoken by an unknown voice). Patients who hallucinated differed from control patients selectively in the last of these tasks, and by failing to show normal activation of left middle temporal gyms and rostral supplementary motor areas.

This empirical study shows mismatch negativity, N2, and P300 abnormalities in 20 patients receiving medication and 11 drug-withdrawn patients, compared with 11 control individuals, mismatch negativity significantly predicted P300 deficit. The theoretical bases of the mismatch negativity abnormality are discussed, particularly the implications of diffuse impairment of cortical connectivity, possibly mediated by glutamatergic mechanisms, with functional ramifications, including findings of 'working memory' deficits, and clinical expression including disorganization symptoms and the 'deficit syndrome.'
