Recommendations for the use of live attenuated varicella vaccine

Committee on Infectious Diseases of the American Academy of Pediatrics: Recommendations for the use of live attenuated varicella vaccine. Pediatrics 1995, 95:791-796. As well as giving the recommendations of the American Academy of Pediatrics for vaccine use (universal use in early childhood and immunization in susceptible older children and adolescents is recommended), this article reviews the data on which these recommendations are based in some detail.

Choo PW, Donahue JG, Manson JE, Platt R: The epidemiology of varicella and its complications. J Infect Dis 1995, 172:706-712. The records of over 250 000 members of a health maintenance programme in New England were analysed to assess the epidemiology of varicella and its complications. The main findings were that incidence rates of varicella in persons under 15 years of age were 20-50% lower than the published figures (probably as a result of under-reporting), whereas rates in individuals of 15 years or older were greater than predicted. The rates of hospitalization for complicated and uncomplicated varicella were many times higher than expected from earlier studies.

J Clin Oncol 1995, 13:1697-1703. The experience at the Children's Hospital of Pittsburgh of 42 cases of VZV infection in 294 children with acute lymphoblastic leukemia is reviewed. The clinical presentation of patients with progressive varicella was dominated by abdominal and back pain rather than skin involvement. Disappointingly, significant morbidity and mortality was observed in patients who had received acyclovir (and VZIG), although delayed diagnoses as a result of atypical presentation may have contributed to the poor outcome.

J Chemother 1995, 7:62-66. An open multicentre study of 53 immunocompetent children aged 3-24 months, who received oral acyclovir therapy either in the first 24 h or within 48 h of the onset of rash. No significant difference in outcome was observed for the two groups. Although of interest as one of the few recent studies of acyclovir therapy in normal children, the study is severely compromised by its small sample size, open design and lack of placebo group.

A randomized, double-blind, placebo-controlled trial. Ann Intern Med 1995, 123:89-96. In this randomized, double-blind multicentre study 419 immunocompetent adult patients with zoster received either famciclovir or placebo for 7 days. Famciclovir was well tolerated, accelerated lesion healing, reduced the duration of acute viral shedding and reduced the duration of post-herpetic neuralgia (defined as pain after skin healing) compared with placebo.

Valaciclovir compared with acyclovir for improved therapy for herpes zoster in immunocompetent adults

Antimicrob Agents Chemother 1995, 39:1546-1553. This large (>1100 adult patients) randomized, double-blind multicentre study compared oral acyclovir 800 mg five times a day for 7 days with valaciclovir 1000 mg three times a day for 7 or 14 days. Valaciclovir taken for 7 or 14 days significantly accelerated the resolution of zoster-associated pain and reduced the duration of post-herpetic neuralgia compared to patients receiving acyclovir. There were no significant differences between the groups in adverse events.

Clin Infect Dis 1995, 20:316-319. The cell-mediated and humoral immune responses of 200 healthy children were compared to one or two doses of live attenuated varicella vaccine. Both cell-mediated immunity and humoral responses were significantly stronger at 1 year in children who had received two vaccine doses 3 months apart. Further studies of the clinical benefit of two rather than one dose of vaccine in children are recommended.

Pediatr Infect Dis J 1995, 14:874-879. This is a follow-up study of 465 healthy children and adolescents (age range 12 months to 17 years) vaccinated with the live attenuated varicella vaccine between 1984 and 1989. The overall seroconversion rate was 94.6%, but varied from 85 to 100% according to the plaque-forming units in the vaccine production lot. Break-through disease occurred in 18.6% of seroconverters during follow-up, but was generally mild. An inverse relationship was seen between the development of break-through varicella and the titre of antibodies to VZV 6 weeks post vaccination.

J Infect Dis 1995, 171:13-17. This study looks at cell-mediated immunity to VZV induced by one or two doses of vaccine in adults and children. The results confirm that the cell-mediated immunity response is diminished in adult vaccinees compared to children. Cell-mediated immunity responses at 1 year were significantly higher in children receiving two rather than one dose of vaccine.

Humoral immune response and VZV-specific lymphoproliferation response to a booster dose of vaccine was assessed in 419 children and adolescents vaccinated with live attenuated varicella vaccine 4-6 years earlier. Of the 419 children, 414 (99%) had remained seropositive after the initial vaccination. The booster dose was well tolerated and effective at boosting antibody titres and lymphocyte proliferation responses.

A decision analysis model was constructed on the basis of published and unpublished data to evaluate the cost-effectiveness of presumptive vaccination versus serological testing of school-age children and adolescents with a negative or uncertain history of varicella. Presumptive vaccination was found to be a relatively cost-effective policy for school-age children, but not for adolescents.
