A useful review indicating that "under-use" of ACE inhibitors may not be due to a lack of medical education or medical neglect but rather restriction to that group of patients incorporated into clinical trials. The evidence that elderly patients or those with so-called diastolic heart failure benefit from treatment is tenuous.

An excellent review questioning the whole aspirin hypothesis and indicating that patients with severe ventricular dysfunction and heart failure may be harmed by aspirin use.

The lower mortality among aspirin users probably reflects intrinsically lower risk rather than therapeutic benefit.

This suggests that the reninangiotensin pathway, rather than bradykinin, is the mechanism of ACE inhibitor benefit.
