This is a very important paper illustrating that meticulous cleaning and disinfection of endoscopic equipment fully eliminated the risk of endoscopic transmission of Helicobacter pylori. Without such precautions, transmission is possible, because heavy contamination of the endoscope and endoscopic equipment is possible after examining a patient with Helicobacter pylori infection.

This important, carefully performed study illustrates the worsening of gastric inflammation in the corpus area during acid-suppressive therapy. This phenomenon fits with the hypothesis that H. pylori can only flourish when the acid-secretory potential of the affected mucosa is rather low or suppressed. More luxurious growth of H. pylori appears to be responsible for aggravating the inflammatory changes.

This is a landmark study, showing quite rapid progression of gastritis toward atrophy in H. pylori-mfected patients during non-stop omeprazole therapy. Apparently, such changes did not occur when the gastric mucosa was normal at the start. The logical consequence of these findings would be that it is mandatory to cure the infection and heal the mucosa is before embarking on long-term acid-suppressive therapy.

In this landmark report of a consensus meeting of the National Institutes of Health in Bethesda, a plea is made to treat all H. pylori-associated peptic ulcer disease patients, both at primary diagnosis and during maintenance therapy, with acid-suppressive drugs, with a therapy aimed at curing the infection. This paper represents a major breakthrough in the overall therapeutic approach for patients with peptic ulcer disease.

Gut 1995; 37: 30-34. This study again confirms some initial data from the Amsterdam study showing that cases with "apparently" recurrent infection with Helicobacter pylori appeared to be due to recrudescent disease, since the same organism was cultured as had been present before. This can readily be done with DNA typing. These investigators used the technique of randomly amplified polymorphic DNA fingerprinting.

2-blocker therapy or prompt endoscopy in management of dyspepsia. Lancet 1994; 343: 811-816. This well-performed study shows that immediate endoscopy is to be preferred over empirical therapy in chronic dyspepsia. This is a highly controversial area, as is illustrated by the fact that several editorial comments followed the publication of this paper. Since this study was carried out, serologic screening for H. pylori infection has become available, which may again change the approach to patients with dyspepsia.
