The incidence of all symptomatic osteoporotic fractures was observed longitudinally in ∼2000 elderly men and women. For subjects older than 60 years, the remaining lifetime risk of symptomatic osteoporotic fracture is >50% for women and >25% for men.

This prospective study examined the risk of hip fracture in ∼10 000 white women aged 65 years or older. Low calcaneal bone density, age, relatively poor health, muscle weakness, poor vision and therapy for thyroid disease, epilepsy and use of long-acting benzodiazepines were all risk factors for subsequent hip fracture. Prior oestrogen use was protective.

In this population-based study of 1600 pre-menopausal women, weight, menopausal status, age and grip strength were independent predictors of bone mineral density of spine and hip and physical activity per se was a determinant of bone mineral density at hip. However, these factors taken in combination explain only 20% of bone density variability.

The Alendronate Phase III Osteoporosis Treatment Study Group: Effect of oral alendronate on bone mineral density and the incidence of fractures in postmenopausal women. New Engl J Med 1995, 333:1437-1443. A new generation bisphosphonate, Alendronate, was compared with calcium alone over three years in 994 women with postmenopausal osteoporosis. The women receiving alendronate increased bone density at all sites relative to control by up to 8.8%. Moreover, the number and progression of vertebral deformities and the number of peripheral fractures were all significantly fewer in treated women.
