In fresh tumor material of 55 patients with vulvar cancer, 33 (60%) contained HPV DNA as detected with polymerase chain reaction using a consensus L1 primer. In a multivariate analysis, controlling for tumour diameter, age grade and nodal metastases, HPV status remained an independent prognostic factor. This result is remarkable because when studies are presented on the prognostic factors for survival in vulvar cancer using multivariate analysis, nodal status is always the most significant predictor.

Cancer 1995, 75:2885-2890. In a multivariate analysis in 66 patients with positive groin lymph nodes and squamous cell cancer of the vulva, extracapsular growth of lymph node metastases was the most significant independent predictor for poor survival. Also, more than 50% replacement of the nodes by tumour and two or more positive nodes predicted poor outcome.

In a multivariate analysis, with primary tumour parameters and nodal parameters taken into account, extranodal growth was not associated with survival. Patients with bilaterally positive nodes had a relative risk of dying of 9.69 compared with patients without lymph node metastases.

A computed tomography scan was performed in 100 women to measure the depth of the groin nodes. It was found that the mean depth of the femoral nodes was 4.5 cm. The authors calculated the dose that would have been received when 5000 cGy was administered to a depth of 3 cm (as in GOG protocol 88). Almost 50% of the patients would have recieved 3000 cGy or less.

With a combination of external beam radiotherapy using 18 MV photons through a anterior-posterior posterior-anterior field, 4500-5040 cGy was administered to 19 patients with locally advanced vulvar cancer. Boosts were given to 10 patients, with either implants or electrons. The inguinal node region was boosted with electrons to a total dose of 3654-5000 cGy. In 10 patients additional surgery was performed. After a median follow-up of 34 months, a local control of 74% was achieved. Only two patients died of disease because of distant metastases.

Three groin recurrences were noted in a superficially negative groin. One contralateral groin recurrence was noted after a negative ipsilateral inguinal groin dissection.

Int J Gynecol Cancer 1995, 5:128-133. Lymphangiograms of 73 patients were studied to define the area medial from the anterior superior iliac spine not containing inguinal lymph nodes. They concluded that by conserving 15-20% of the fatty tissue medial from the spine there was a 99.8% chance of complete nodal clearance.
