Eur Heart J 1994, 15:68-75. In a prospective echocardiographic study in 124 homosexual men in London followed for an average of 1.6 years, the authors found a 20% incidence of left ventricular dilatation or dysfunction in the subgroup of 101 patients with AIDS. Of these 20 patients, eight had signs and symptoms of congestive heart failure.

J Am Coll Cardiol 1994, 24:1018-1024. This prospective, observational, echocardiographic study followed 136 patients from different HIV risk groups, for an average of 1.1 years. Acute, global left ventricular dysfunction occurred in seven patients with AIDS, and six died a primary cardiac death. Histopathologic examination at necropsy in revealed myocarditis or interstitial edema/fibrosis in five patients with cardiomyopathy.

J Am Coll Cardiol 1994, 24:1025-1032. An important study demonstrating that cardiotropic virus infection and myocarditis may be important in the pathogenesis of symptomatic HIV-associated cardiomyopathy. Of 37 patients with left ventricular dysfunction who underwent endomyocardial biopsy, the majority were found to have myocarditis by histology and immunohistology. In situ hybridization studies using antisense riboprobes for HIV-1 and cytomegalovirus immediate early genes yielded specific hybridization within myocytes.

Didanosine therapy was not associated with the development of cardiomyopathy. The association between use of antiretroviral therapy with zidovudine and the development of cardiomyopathy is well demonstrated.

In a retrospective series of 29 patients who underwent pericardial window procedures for pericardial effusions between 1986 and 1994 in urban New York, 24% of cultures were diagnostic, although in 94% of cases there was no change in clinical management based on operative results. At 22 weeks of follow-up, mortality was 100%, indicating a grave prognosis for HIV-related pericardial effusions requiring operative pericardial drainage.
