A very important paper, because it illustrates the tremendous differences in transfusion practice and volume replacement documented for routine elective surgery therapy performed in 43 European teaching hospitals. Specific interest is focused on the efforts made in the different hospitals to avoid homologous transfusion.

This study has shown that most of the RBC transfusions in critically ill patients are given with the aim to keep the haemoglobin concentration in a preset range rather than to meet actual physiological needs of the patients. Furthermore, daily phlebotomy (blood sampling for laboratory tests) significantly contributed to the fall in haemoglobin concentration observed in this study.
18. Hébert PC, Wells G, Marshall J, Martin C, Tweeddale M, Pagliarello G, Blajchman M, for the Canadian Critical Care Trials Group: Transfusion requirements incritical care - a pilot study. JAMA 1995, 273:1439-1444. This very important clinical study shows that a lower transfusion trigger (haemoglobin concentration of 7.0-9.0g/dl) results in a reduction on the number of RBC units given per patient without any apparent negative effects on patient outcome.

J Trauma Injury Infect Crit Care 1995, 38:780-787. This publication is one in a series of reports on the importance of physiological endpoints for resuscitation in trauma patients. The values were obtained while the haemoglobin concentration was maintained at greater than or equal to 10g/dl.

JAMA 1994, 271:777-781. The most recently published guidelines aiming at restriction of use of blood products in the management of postoperative bleeding problems. Liberal use of blood products cannot be justified with respect to hazard/benefit.

The authors give an excellent review of the literature on the effects of transfusion on the immune system. Concise information about the mechanisms of the effects of homologous transfusion on the host's immune system is provided.
