The authors present the results of a survey of over 38 000 households on the use of outpatient psychotherapy from the 1987 National Medical Expenditure Survey. The authors show that Americans made an estimated 71.5 million outpatient visits at a cost of $4.2 billion in this year, accounting for about 8% of all out-patient medical costs. Women, white individuals, middle-aged people, the separated and divorced, and college graduates had a greater likelihood of using psychotherapy, although income was not related to use.

The authors indicate that long-term users of psychotherapy are only approximately 16% of users but account for about 63% of total psychotherapy expenditures. Long-term users of psychotherapy are disproportionately women, white, separated and divorced, college graduates, and high income individuals. In contrast, elderly people, black people, and individuals who have not attended college are particularly unlikely to receive long-term psychotherapy.

The use of veteran and nonveteran mental health services by 1700 male veterans in four ethnic groups was examined. Contrary to expectations, no ethnic differences existed in the use of veterans services, whereas black people and Mexican veterans were less likely to use nonveteran services, after controlling for other factors.

Health Serv Res 1995, 30:319-340. A panel study of nearly 800 patients screened for depression in three urban areas in the USA. Patients in prepaid plans had considerably fewer mental hearth visits than patients in fee-for-service plans, and patients who switched out of either type of plan had fewer visits than patients who remained in their plan.

The authors compared patterns of mental health use in the USA and Israel to find whether the lack of financial barriers to treatment in Israel elevates mental health treatment among elderly people. Contrary to expectations, elderly individuals in both countries showed low rates of mental health treatment and rates in Israel were even lower than those in the USA.

Data from the Piedmont, North Carolina, USA, site of the ECA were used to investigate how insurance coverage for mental health services affected service use among people with and without a measured diagnosis. The authors showed that the impact of insurance coverage on use of services is strongest among people with a measured diagnosis, indicating that restricting coverage for mental health services may have a stronger impact in reducing treatment among people who most need it.
