The Ischemic Optic Neuropathy Decompression Trial Research Group: Optic nerve decompression surgery for nonarteritic anterior ischemic optic neuropathy (NAION) is not effective and may be harmful. JAMA 1995, 273:625-632. This randomized controlled clinical trial provided the first absolute evidence that the natural history of NAION is better than most people had suggested, with over 40% of patients experiencing spontaneous improvement in visual acuity. The study results also indicated that decompression of the optic nerve sheath is an inappropriate treatment for most, if not all, cases of this condition.

The authors of this retrospective study found that over 30% of patients with arteritic ischemic optic neuropathy experienced improvement in visual function when treated early in the course of visual loss with high-dose systemic corticosteroids. This is one of the first studies that has shown that patients who lose vision in the setting of giant cell arteritis have some potential for visual improvement.
