Three thoracic histopathologists reviewed 74 tumors using the Müller-Hermelink system. They found marked intra- and interobserver variability, with only a unanimous agreement on 26 cases. They felt this variability was due to the difficulty in applying the system. The investigators also found that the survival of patients with mixed tumors was worse than cortical tumors, which is in disagreement with other reports. They found stage to be the most important prognostic factor.

The first study to document survival differences among histological subtypes of the Müller-Hermelink system. Of 116 patients, 84 were stage I or II, 28 were stage III, and four were stage IV. There was a 50% 10-year survival rate for patients with WDTC and a 100% 10-year survival rate for all other subtypes. This very high survival for all the other subtypes except WDTC makes the study difficult to interpret.

This report describes the isolation of a defective Epstein-Barr virus gene product from a thymic carcinoma. The gene product is homologous to other transcription factors, and it is felt to play a role in activating replication of latent virus. Histologically, the tumor was identical to another Epstein-Barr virus-related carcinoma, the undifferentiated nasopharyngeal carcinoma.
