1. Hsu HC, Wu JJ, Jim YF, Chang CY, Lo WH, Yang DJ: Calcific tendinitis and rotator cuff tearing: a clinical and radiographic study. J Shoulder Elbow Surg 1994, 3:159-164. Age and gender distribution of these disorders are different in Asian people from their distribution in whites. The 82 patients studied included 61 men. Fifty-six patients were older than 60 years. Arthrographically, 17 patients had a complete rotator cuff tear. An acromial or acromioclavicular spur was found 26 times.

3. Loew M, Jurgowski W, Mau HC, Thomspn MJ: Treatment of calcifying tendinitis of rotator cuff by extracorporeal shock waves: a preliminary report. J Shoulder Elbow Surg 1995, 4:101-106. Twenty patients were followed up for 12 weeks. Fifteen had a marked reduction of symptoms. Seven patients had a complete disappearance, and five had a partial disintegration of the deposit. Morbidity was low overall.
4. Bunker TD, Anthony PP: The pathology of frozen shoulder. J Bone Joint Surg Br 1995, 77:677-683. Histologic and immunohistologic biopsy findings in 12 patients seem to indicate that frozen shouder is a disease like Dupuytren's contracture.
5. Bunker TD, Esler CNA: Frozen shoulder and lipids. J Bone Joint Surg Br 1995, 77:684-686. A comparison was made between 43 patients with frozen shoulder and 43 age- and gender-matched controls. A statistically significant elevation of cholesterol and triglycerides was found in the frozen shoulder group.
6. Nobuhara K, Supapo AR, Hino T: Effects of joint distension in shoulder disease. Clin Orthop 1994, 304:25-29. In 486 of 1125 patients with a painful stiff shoulder and an obliteration of the subscapular bursa, the bursa was distended successfully by arthrography and forced mobilization. Dramatic pain relief was immediate.
7. Pollock RG, Duralde XA, Flatow EL, Bigliani LU: The use of arthroscopy in the treatment of resistant frozen shoulder. Clin Orthop 1994, 304:30-36. Arthroscopy following manipulation with the patient under anesthesia allowed identification and treatment of associated pathology; it yielded satisfactory results in 25 of 30 patients.

11. Lee DH, Niemann KW: Bipolar shoulder arthroplasty. Clin Orthop 1994, 304:97-107. Bipolar shoulder arthroplasty appears to be an alternative to primary arthroplasty in patients with rheumatic diseases, providing the subacromial arch is intact and no deltoid abnormality is present.

Twenty-two of 32 patients with chronic pain 6 months after a single nondislocating trauma had a labral lesion. Although magnetic resonance imaging is valuable, a preoperative arthroscopy is necessary.
14. Sonnabend D: Treatment of primary anterior shoulder dislocation in patients older than 40 years of age. Clin Orthop 1994, 304:74-77. Of 53 patients, 13 had significant associated lesions; 11 had rotator cuff tears and two had a tear plus a neurologic deficit.
15. Edwards DJ, Hoy G, Saies D, Hayes MG: Adverse reactions to an absorbable shoulder fixation device. J Shoulder Elbow Surg 1994, 3:230-233. Adverse effects were reported for a polyglyconate implant used for stabilization of the glenohumeral joint or repair of superior labral lesions necessitating arthroscopic lavage and debridement. This complication occurred in six of more than 100 procedures.

Most degenerative changes take place in the attachment zone. They are accompanied by alterations in collagen characteristics that not only give rise to the clinical syndrome of enthesopathy but also may be responsible for the impairment of biomechanical properties.

Infraspinatus degeneration, which can occur in the absence of an infraspinatus tear, is associated with a poor outcome of supraspinatus repairs. Whereas infraspinatus fatty degeneration never regresses, that of the supraspinatus can regress after successful surgery.

19. Hattrup SJ: Rotator cuff repair: relevance of patient age. J Shoulder Elbow Surg 1994, 4:95-100. Report on 53 patients older than 65 years and 35 younger than 65 years. Larger tears were more frequent in patients older than 65 years. Of the patients younger than 65, 88.6% had excellent results compared with 77.4% in the older group. Results in small or medium tears were better (89.2%) than in larger or massive tears (80.4%).

Ultrasonography could not reliably differentiate between partial- and full-thickness tears. Arthrography had a greater sensitivity, a greater accuracy, and a greater predictive value for positive test results than ultrasound. The specificity was equal for both.

Results of repair are satisfactory as long as the head remains centered, even in the presence of a persisting defect of the supraspinatus. Defect of the infraspinatus leads to lack of centering and therefore to poor results.

In the presence of massive irreparable tears, only the margins are repaired and superior defects are left unrepaired. In this study all but one patient improved and six of 12 had excellent or good results.

Report of 50 patients (53 shoulders) followed up for an average of 6.5 years. With proper rehabilitation, adequate decompression of the subacromial space, anterior acromioplasty, and debridement of the massive tear, relief of pain and restoration of function can be expected.

Long-term results of anterior acromioplasty alone in the treatment of full-thickness tears are not good. This conclusion is contrary to that reached by Levy et al. (Arthroscopy 1991, 7:8-19) after a short-term study of the same patient group.

In the presence of complete ruptures of the cuff, an anterior acromioplasty is recommended only for older, sedentary persons. Its main effect is antalgic.
31. Liu SH: Arthroscopically-assisted rotator-cuff repair. J Bone Joint Surg Br 1994, 76:592-595. In this multicenter study, 44 patients underwent an arthroscopically assisted repair of full-thickness rotator cuff tears and were followed up for an average of 4.2 years. Eighty-four percent had a good or excellent result, and 88% were satisfied. The size of the tear was a determining factor in the outcome.
32. Walch G, Nove-Josserand L, Levigne C, Renaud E: Tears of the supraspinatus tendon associated with "hidden" lesions of the rotator cuff interval. J Shoulder Elbow Surg 1994, 3:353-360. Exploration of the rotator cuff interval in 116 apparently isolated supraspinatus lesions revealed lesions of the coracohumeral ligament, the superior glenohumeral ligament, and the superior portion of the subscapularis in 19 patients. Computed tomographic arthrography can reveal these lesions in 85%. Surgical repair is recommended.
33. Nobuhara K, Hata Y, Komai M: Surgical procedure and results of repair of massive tears of the rotator cuff. Clin Orthop 1994, 304:54-59. Ninety-three percent of 187 patients who underwent a surgical repair had an excellent or good functional result after an average of 6 years and 9 months. Whereas 38% of patients with a tendon-to-tendon repair complained of pain, only 18% with a tendon-to-bone repair were symptomatic.
34. Groh GI, Simoni M, Rolla P, Rockwood CA: Loss of the deltoid after shoulder operations: an operative disaster. J Shoulder Elbow Surg 1994, 3:243-253. Report on 36 patients who suffered irrevocable pain and impairment of function secondary to denervation (n = 3) or detachment (n = 33). There is no cure for this surgical complication. Treatment is nonspecific and supportive.

Average follow-up time was 4 years.
