Nedocromil sodium was released for use in the United States in 1993. Despite its unpleasant taste, twice daily nedocromil sodium is useful for mild and moderate asthmatics. In this study patients who received nedocromil sodium had fewer symptoms and less need for rescue medications than patients treated with placebo. There was no difference in forced expiratory volume in 1 second between groups, but patients in the placebo group used 23% more β-agonists, perhaps artificially diminishing the difference between groups.

Although BDP, 100 mg/d (2 puffs) cost $376 a year more than albuterol alone in this study, $454 a year per patient is saved on extra outpatient visits, medications, and hospitalization costs, for an overall saving of $78 a year per patient. This cost savings is an underestimate of the true cost effectiveness of ICs, as it does not take into account testing and radiographs during the hospitalizations or the savings in indirect costs.

Many comparative studies of IC preparations compare an equal number of puffs of canisters delivering different doses per actuation or compare inhaled medications delivered in different manners. This is one of a few studies comparing medications over a full dosing range (400 to 4000 μg/d) on a microgram-to-microgram basis. For any given dose, BDP, triamcinolone acetonide, and fluticasone propionate were equivalent in their dose-dependent ability to reduce sensitivity to antigen inhalation challenge and in their capacity to reduce urinary free cortisol.

Twenty-one asthmatic children (mostly prepubertal) were treated with low- or mid-dose inhaled BDP with a spacer device for 7 to 16 months. Bone densitometry was measured twice and compared with normal curves developed by measuring bone density on 107 normal children. The study group demonstrated an increase in bone density as expected with age.

Ann Allergy Asthma Immunol 1996, 76:234-238. The benefits of spacer devices to enhance drug delivery are well known. In this paper, the authors demonstrate that spacer use is associated with a decreased risk for reduction in urinary free cortisol excretion. Forty-seven percent of patients using a metered-dose inhaler without a spacer had reduced excretion as compared with 8% of those who used a spacer. As a whole, the former group's 24-hour urinary free cortisol excretion was significantly less than that of the spacer or nontreated control groups. The spacer group did not differ from the control group.

BMJ 1995, 310:1225-1228. Over 100 children ages 9 to 11 were monitored for 13 months using diary card respiratory symptom and peak expiratory monitoring. Parents telephoned when symptoms occurred and specimens were obtained for viral analysis. Viruses were reported in 85% of episodes of upper respiratory symptoms, cough, wheeze, and fall in peak expiratory flow rate.
