Three sunburns per year was significantly associated with malignant melanoma (RR 1.9). There was a significantly increased risk associated with sunburns after 19 years of age (RR 2.2); five or more sunburns versus none, but no higher risk for individuals who had experienced severe sunburns during childhood.

There was a significant increase in relative risk of 17 for developing melanoma.

This study is adjusted for age, sex, hair color, and holiday weeks in sunny resorts; cutaneous melanoma was associated with the regular use of common (RR = 1.5) and psoralen (RR = 2.28) sunscreens. The use of sunscreens, especially psoralen sunscreens, was associated with higher density of nevi and freckles of the skin.

The frequency of melanomas was increased 1000-fold or more in XP patients younger than 20 years of age. The site distribution of melanoma and non-melanoma skin cancers was like the general population, with a higher proportion of face, head, or neck and a lower proportion on the trunk, particularly in males. The data suggests that DNA repair plays a major role in the prevention of skin cancers in the general population, and that sunlight exposure is responsible for the development of melanoma as well as nonmelanoma skin cancers in patients with XP, although different mechanisms may be involved for the two types of skin cancers.

Various sunscreens offered minimal protection against local and systemic suppression on contact hypersensitivity in mice after chronic treatment with suberythemal, solar-simulated ultraviolet radiation. The most effective sunscreen had immunoprotective factors between 1 and 1.13, whereas their sun protection factor for mice was 4.0 and 7.0. respectively.
