Blood 1995, 85:2303-2307. Because stem cell factor is now being studied in phase III trials as an adjunct to G-CSF in the mobilization of peripheral blood progenitor cells for the support of high-dose chemotherapy, the long-term engrafting potential of these cells is of obvious importance. These authors report on their serial transplantation experiments in mice that support the conclusion that these cells have durable engraftment potential.

J Cancer Res Clin Oncol 1995, 121:338-342. Previous studies have suggested that serum levels of stem cell factor decreased in patients with myelodysplastic syndromes. This in vitro study addresses the potential of stem cell factor to overcome the hematopoietic defects associated with myetodysplastic syndromes. The authors show that stem cell factor and erythropoietin are synergistic in stimulating erythropoiesis in myelodysplastic syndromes such as refractory anemia, but that this responsiveness is lost in patients with refractory anemia with excess of blasts. They also demonstrate that stem cell factor may synergize with myeloid growth factors in supporting myelopoiesis in some myelodysplastic syndrome cells.

The authors show that stem cell factor and erythropoietin are synergistic in stimulating erythropoiesis in myelodysplastic syndromes such as refractory anemia, but that this responsiveness is lost in patients with refractory anemia with excess of blasts. This study suggests that if stem cell factor has clinical utility in the treatment of patients with myelodysplastic syndrome, it may be as an adjunct to therapy with other lineage-specific cytokines and that careful attention should be paid in these trials to the different subtypes of myelodysplastic syndromes, because they may respond differently.

Two major advances in hematology over the past year have been the cloning of the mpl ligand and the demonstration that this factor is the primary regulator of platelet production. One issue for the clinical development of thrombopoietin will be its efficacy and rate of response in patients with diminished stem cell reserves. The observation by these authors that stem cell factor may synergize with thrombopoietin in stimulating the proliferation of colony-forming unit megakaryocytes may have future clinical importance for optimizing the clinical utility of that molecule.

Pretreatment for 5 days with stem cell factor followed by the injection of leukemia inhibitory factor results in a markedly enhanced thrombopoietic response to leukemia inhibitory factor. With the recent purification and cloning of thrombopoietin. the observation that stem cell factor may enhance the thrombopoietic response to stimulatory cytokines may have future clinical importance.
