This report of the Musculoskeletal Tumor Society has laid the scientific groundwork for limb-salvage surgery. Among the 227 patients with osteosarcoma restricted to the distal femur, the group treated with limb-salvage surgery was not significantly different in metastatic rate or survival at median 5.5-year follow-up as compared with the above-knee amputation and hip disarticulation group.

J Pediatr Orthop 1994, 14:781-787. A prospective analysis of 119 MRI studies in 41 patients were evaluated. The authors found that T2-weighted images were the most useful in assessing extent of soft-tissue involvement and T1 imaging for documenting the bone marrow changes. MRI added significantly to the evaluation of primary bone tumors by computed tomography, radiographs, and bone scans.

MR imaging findings in recurrent primary osseous Ewing sarcoma
-
99mTc medronate methylene diphosphonate skeletal scintigraphy, and radiographs. MRI identified nine of the 11 recurrences, bone scans only four of 11 cases, and plain radiographs two of 10. These findings of changes in signal intensity, increased extent of abnormal marrow signal on T1-and T2-weighted images, and identification of a new soft tissue mass suggest that MRI is valuable in the routine follow-up of Ewing's sarcoma.

Schiller C, Windhager R, Fellingerr EJ, Kotz R: Extendable tumor endoprosthesis for the leg in children. J Bone Joint Surg Br 1995, 77:608-613. Six patients who have endoprostheses after malignant bone tumor resection have reached skeletal maturity and are the subject of this study. All patients were free from disease at follow-up; five had osteosarcoma and one had Ewing's sarcoma. The mean length gained was 13.15 cm, requiring 53 planned procedures. Functional evaluation showed excellent and good results, though stress-shielding at the site of anchorage was more pronounced than in adults. This stressshielding improved in three patients with the addition of autologous and allograft bone grafting. Deep infections occurred in three patients, requiring debridement, immediate reimplantation of the autoclaved endoprosthesis, and long-term administration of antibiotics.

Alman BA, Krajbich JI: Massive allografts in the treatment of osteosarcoma and Ewing sarcoma in children and adolescents. J Bone Joint Surg Am 1995, 77:54-64. The results in 26 patients with osteosarcoma and Ewing's sarcoma were examined after resection and allograft reconstruction. This heterogeneous population included upper extremity lesions as well as lower extremity tumors. The rate of infection was 12% and was believed to be partly due to lack of an adequate soft tissue envelope, which the authors emphasize is of paramount importance in the prevention of infection. They state that they routinely perform primary muscle (gastrocnemius or latissimus dorsi) transfers. They conclude that allograft reconstruction is associated with a higher rate of complications in children than in adults. They found it more useful in the upper extremity where the limb length at maturity is not as great a concern.
