In this anecdotal study, seven patients with idiopathic subglottic stenosis are presented, six of whom had failed surgical intervention. Monitoring of pH was performed in five of these patients using three probes (pharyngeal, proximal, and distal). The study was positive for distal esophageal reflux in all of these patients but showed proximal reflux in only three and no documented episodes of pharyngeal reflux. All patients improved when antireflux measures were added to their therapeutic regimens.

Am J Gastroenterol 1995, 90:1785-1790. Using prolonged pH monitoring and controling for the pharmacologic half-life of nicotine, this study shows that cigarette smoking significantly increased the percentage of time that the pH was lower than 4 during the 24-hour study period. This increase occurred predominantly during the day while the patient was in the upright posture and was associated with a 114% rise in subjective complaints of heartburn.

This provocative study uses both prolonged pH and bilirubin monitoring to demonstrate significantly higher esophageal bilirubin exposure in patients with Barrett's esophagus as compared with those with erosive esophagitis and no injury. The authors also show that the majority of esophageal bilirubin exposure occurred when the pH was between 4 and 7. These findings may ultimately explain why a small but significant proportion of cases fail to resolve on acid-suppression therapy alone.

Using a rat model, mixed reflux was shown to cause greater esophageal mucosal damage than acid reflux alone. The source of superoxide ions appears to be lipid peroxidation. Superoxide dismutase given prior to experimentally induced reflux prevented esophagitis. The role of glutathione in this process is proposed but less firmly established.

Although the authors demonstrate that this modality remains an excellent tool for characterizing patterns of reflux in patients with severe esophagitis, they also show that it becomes less effective in patients with mild to moderate esophageal reflux.

This study examines the clinical response of 182 patients to various levels of antireflux therapy and serves to illustrate the point that pH monitoring may not be necessary in the initial evaluation of patients with reflux laryngitis. However, the failure of the authors to document reflux using accepted methods prior to instituting therapy leaves the results suspect from a scientific point of view.
