J Intern Med 1995, 237:105-115. A superb clinical review on a large number of interventionally treated patients. The authors treated 18 intracranial vessels with PTCA and had a 75% successful vessel opening rate The authors also used PTCA to treat symptomatic vasospasm secondary to subarachnoid hemorrhage in 28 patients with 99 symptomatic arteries. All cases had angiographic documented improvement in vessel diameter. The authors then describe their experience using intra-arterial thrombolytic therapy in 24 patients treated within 12h of stroke. Successful clot lysis occurred in 80% of the cases with 62% showing neurologic improvement.

An important clinical study of 19 patients treated with PTCA. The procedure was successful in 13 cases with the average stenosis before treatment of 83±8% being reduced to 35±17% post-treatment. The patients with the greatest improvements in symptoms were the ones who showed the most improvement in cerebral blood flow measured with SPECT scans. To reduce the risk of emboli associated with PTCA, the author describes a new coaxial balloon catheter system.

The authors describe their experience using PTCA in 22 intracranial vessels in 17 selected patients. All patients had at least 70% stenosis and were symptomatic despite optimal medical therapy. Stenosis prior to angioplasty averaged 72±8% with an improvement seen after angioplasty to 43±23%. There were two angioplasty related strokes resulting in a per vessel complication rate of 9%.

The authors describe thrombolysis results from a nonrandomized consecutive trial using either direct intraarterial therapy, local intercarotid therapy, or intravenous therapy in 90 patients treated within 8 h of an acute stroke. The rate of recanalization in the direct intraarterial group was 84%, compared with just 11% in the local thrombolytic group. For patients with MCA stroke, the rate of recovery in the intraarterial group was 44% compared with 19% in the locally treated group and 33% in the intravenous group.

Stroke 1994, 25:298-303. The authors describe direct intraarterial urokinase treatment in 34 patients with negative head computed tomography scans treated within 6 h. They report a 94% rate of vessel recanalization and a 65% rate of neurologic recovery at 1 month. An important finding of this study is that in the 21% of patients who had hemorrhagic transformation, the SPECT scans performed prior to treatment showed significantly lower cerebral blood flow compared with the patients who did not develop hemorrhages.

AJNR 1994, 15:1817-1822. In this paper direct intraarterial treatment is described in 13 patients with acute ischemic stroke. Successful thrombolysis was obtained in 77% of the patients. Nine out of 13 of the patients showed major neurologic improvement as measured by the National Institutes of Health stroke scale. An important point of this paper is that rather than limiting the patients to stroke within 6 h, the authors used a negative computed tomography scan as the criteria for entrance. Several of the basalar artery strokes were treated after 24 h of symptom onset.
