Blood 1995, 85:2643-2653. In this report, the authors updated the results of their randomized trial comparing chemotherapy alone and ATRA followed by chemotherapy in newly diagnosed APL. With a minimum follow-up of 21 months, the incidence of relapse remained significantly lower, and actuarial survival and event-free survival remained higher after ATRA followed by chemotherapy. They also updated results of an earlier pilot study of ATRA followed by chemotherapy, which after a minimum follow-up of 38 months still gave better results than chemotherapy alone.

Blood 1994, 84:3843-3849. The authors critically review the necessity of adding chemotherapy in order to prevent the ATRA syndrome in APL and conclude that chemotherapy is rarely required. They show that expression of CD13 on blasts, rather than leukocyte counts, can be a predicting factor of ATRA syndrome, and that patients with a PML-RAR breakpoint in bcr3 region may have a higher relapse risk.

Bone Marrow Transplant 1994, 14:293-303. The authors reviewed the outcome of allogeneic and autologous bone marrow transplantation in first or second complete response in APL, in patients who had received chemotherapy alone as a first-line therapy. It appears that these procedures, in first complete response are not superior to ATRA followed by chemotherapy except perhaps in some high-risk patients, but that they probably represent the only chance of achieving cure in second complete response.
