High levels of conjugated metabolites of midazolam are shown to be responsible for prolonged sedation in patients with renal failure, since the parent drug as well as the unconjugated main metabolites were well below the therapeutic range. Quick arousal occurred after administration of flumazenil.

It can be shown that duration and total dose are significantly correlated with the incidence of withdrawal reaction. A total dose of greater than 2.5 mg/kg or a duration of infusion of greater than 9 days was 100% predictive of withdrawal symptoms.
