Using a rat model of (heterotopic) aortic transplantation, the authors demonstrate that in allograft aortas similar changes occur as would be expected in human allograft atherosclerosis, including adventitial inflammation and intimai thickening. No such changes even with cytomegalovirus infection occur in syngeneic transplants. There is upregulation in several growth factors that are implicated in altering intimai thickness and regulating smooth muscle cells of the aorta.

Circulation 1994, 90:1969. In the same rat aortic transplantation model, the authors began ganciclovir treatment either 1 day prior to cytomegalovirus infection (prophylaxis group) or at day 7 or later (treatment group) and continue treatment for 14 days. Rats in the prophylaxis group had a significant reduction in inflammatory cell proliferation in the adventitia and intima and a reduction in interleukin-2 receptor positive cells. The authors also demonstrated an effect of ganciclovir on the proliferative responses of even uninfected smooth muscle cells that is still under investigation.

Thorax 1995, 50:9. A prospective study on 23 heart transplant and nine lung transplant recipients using weekly surveillance for antigenemia using a mouse monoclonal anticytomegalovirus antibody with more than 50 positive polymorphonuclear leukocytes per 200,000 cells defined as high positive. This test was able to accurately predict symptomatic cytomegalovirus syndrome or invasive disease and became positive significantly before the onset of symptoms.

In contrast to a prior study using ganciclovir for 28 days, here no benefit was seen in the larger cytomegalovirus seropositive recipient group whereas benefit was seen in the high-risk seropositive donor - seronegative recipient group. The absence of OKT3 induction perhaps explains the relatively low incidence of cytomegalovirus in the seropositive recipient placebo group (about 25%).

A randomized controlled, multicenter trial that demonstrated that as little as 2.5 mg/kg/d of intravenous ganciclovir during antilymphocyte antibody therapy significantly decreased the incidence of cytomegalovirus disease. Since the completion of this study, this approach has been found to be equally effective in cardiac allograft recipients at a slightly higher dose of 5 mg/kg/d.

J Heart Lung Transplant 1995, 14:214. This study represents a large retrospective review of the Mayo Clinic experience with 288 liver, 44 heart, 20 lung, and 37 kidney-pancreas transplant recipients from 1985 to present. Fourteen adult cases of PTLD were diagnosed during the time period. Only three of 367 patients seropositive for EBV developed PTLD versus 11 of 22 patients seronegative for EBV pretransplantation who developed PTLD; most of these 11 patients seroconverted posttransplantation. Disease tended to occur within the first 2 years after transplantation; localized disease to the allograft or to nodes was far more responsive to therapy than extranodal disease or CNS disease.

In this brief case report, the patient, who was treated with steroids, developed acute rejection on day 10. Several days later an fever of undetermined origin syndrome developed that culminated on day 33 with a new pencardial effusion. Endomyocardial biopsy confirmed T. gondii infection, which responded to therapy. The importance of biopsy to make the diagnosis in this case is reviewed.

The authors report the first case of parvovirus B19 - induced pneumonitis in a pediatric heart transplant recipient. The patient developed rash, fever, and infiltrates 2 weeks postoperatively; BAL was negative for other pathogens and IgM against parvovirus B19 was positive. No parvovirus B19 DNA was isolated, however.

Although the vast majority of nocardiosis is pulmonary with or without metastatic spread to other sites, especially the CNS, the authors of this case report present a locally invasive case related to the intramuscular injection of pain medications. This also represents the first case of N. farcinica infection in a transplant patient, the majority of which are due to N. asteroides or N. brasiliensis.
