The authors report on fourteen pediatric patients with pulmonary hypertension treated with inhaled NO. Overall, one third had a substantial response to oxygenation, one third had an undesirable response, and one third had no response in terms of improvement.

Pediatrics 1994, 94:715-718. A lack of response to inhaled NO in nine patients with PPHN secondary to pulmonary hypoplasia (eight with CDH) was reported. All patients went to ECMO. Five infants in whom pulmonary hypertension recurred after ECMO did respond to NO inhalation.

Thirteen consecutive neonates with severe PPHN were treated with inhaled NO at doses of 20 ppm or less. Responses were rapid in six patients, intermediate in three, delayed in three, and absent in one.

Six of seven pediatric patients with postoperative pulmonary hypertension had a marked improvement in oxygenation following inhaled NO administration. Early NO weaning failed and they received NO treatment for up to 16 days, with resolution of the pulmonary hypertension.
