Observations of patients treated with Nevirapine (an NNRTI) and protease inhibitors indicate that plasma virions and virus producing cells are cleared rapidly, with half lives of less than 2 days. Similar findings are found with both classes of drugs, making artifactual effects of drugs on virion clearance unlikely.

J Infect Dis 1995, 171:1411-1419. Plasma viral load and CD4 counts were followed after initiating treatment with 3TC. The baseline level of viremia on treatment is less than pretreatment levels despite the emergence of drug resistant virus. This suggests that the resistant genotype may have a partially sensitive phenotype or that the resistant virus has a diminished replicative capacity.

The authors use mathematical models and observations of chromosomal damage after radiation therapy to derive estimates of proliferation rates of naive and memory class human T lymphocytes. They deduce that naive lymphocytes divide once every 3.5 years and memory lymphocytes divide every 22 weeks. Decay of lymphocytes with stable chromosome damage is very slow.

Analysis of a large cohort indicates that total T cells remain relatively stable through most of the course of HIV infection, which is consistent with regulation of total T cell number. A steady decline in total T cell count begins approximately 18 months prior to the onset of AIDS, suggesting a failure of 'blind T cell homeostasis'.
