This randomized trial demonstrates that attenuated m-BACOD resulted in inferior control of the lymphoma but less toxicity and equivalent survival compared with conventional dose m-BACOD given with granulocyte-macrophage colony-stimulating factor. The equally poor results in both arms of the study leaves substantial room for improvement.

Sparano JA, Hu X, Wiernik PH, et al.: A pilot trial of infusional cyclophosphamide, doxorubicin, and etoposide plus didanosine and granulocyte colony stimulating factor in patients with HIV-associated non-Hodgkin's lymphoma. J Clin Oncol 1996, in press. This study demonstrates that it was feasible to combine didanosine with the infusional CDE regimen, and that didanosine ameliorated leukopenia, anemia, and thrombocytopenia. Didanosine did not ameliorate the CDE-induced decrease in CD4 cells. HIV load increased in the majority of patients and was not abrogated by didanosine. Finally, significantly greater myelosuppression was observed in patients with low baseline CD4 counts and lymphomatous marrow involvement.
