Due to methodologic shortcomings at the time of its initiation, the trial could not classify patients according their immunologic (autoreactive) or viral status (PCR/in situ hybridization-positive for enteroviruses, cytomegalovirus, Epstein-Barr Virus, adenovirus, borreliosis, and others).

Pinpoints the major shortcomings of the Myocarditis Treatment Trial: inadequate or too stringent criteria for myocarditis due to the lack of immunohistochemistry and limitation to cellular infiltrates as signs of inflammation despite the fact that immunoglobulins, cytokines, and adhesion molecules respresent similarity relevant markers of inflammation. In addition, the duration of the trial appeared too short in the view of the fact that at the time of discontinuation there were nine deaths in the control group but only four deaths in the treatment arm. Also, the selection of cyclosporine as the right drug for this disease is questioned.

The hemodynmic entrance criterion is an ejection fraction of less than 45% in gated blood pool scintigraphy.

In 77 consecutive biopsies enteroviral RNA was detected by in situ hybridization. Treatment with interferon alfa (3 million units subcutaneously every other day for 6 months) was given in four enterovirus-positive patients. In two of them enteroviral RNA was eliminated. All four improved hemodynamically and clinically. In addition, hemodynamics in the enterovirus-positive patients (treated and untreated) showed better improvement than hemodynamics in the enterovirus-negative patients.

Eur Heart J 1995, 16(suppl O):150-157. First prospective, randomized, open-label 3-month treatment trial with interferon or thymic hormones versus conventional heart failure treatment in 40 patients with biopsy-proven myocarditis or with idiopathic dilated cardiomyopathy. Whereas no effect on survival was noted, hemodynamic improvement was significant in the patients treated with interferon and thymic hormones. The results are remarkable despite the fact that patients were not screened for viral persistence and no differentiation was made with respect to the presence of infiltrates (myocarditis vs idiopathic dilated cardiomyopathy).

Mononuclear myocarditis can be induced experimentally by immunization with cardiac sarcoplasmic reticulum ATPase. A monoclonal antibody to this enzyme induced myocytolysis but no infiltrate. This may support the concept of serous inflammation (without infiltrate) or stress the fact that inflammatory infiltrates are not obligatory after a humoral immune assault to the heart that damages the myocardium.
