This trial shows that melanoma nodal metastases are not random events, but rather there is order to the spread of disease to the regional nodes. The authors provide a compelling statistical argument that the SLN is a special node and can reliably determine whether occult metastases are present within the regional basin. This confirms the initial observation of Morton's group (Morton et al., Surg Oncol Clin North Am 1992, 1:247-259) and demonstrates continued enthusiasm for this surgical technique.

Ann Surg 1994, 220:768-774. The sensitivity of the standard methods for pathological evaluation of lymph nodes have long been questioned. The authors described a provocative test using RT-PCR assays for the messenger RNA for the tyrosinase gene. These so-called "submicroscopic" lymph node metastases can be identified in 36% of the histologic node negative melanoma population. Long-term clinical follow-up is necessary to prove clinical correlation.
