The International Bone Marrow Transplant Registry database of over 120 syngeneic marrow transplants for leukemia was used to compare relapse rates with those of HLA-identical sibling BMT. The GVL effect of the allotransplant was quantitated and shown to be greatest for patients with CML, intermediate for acute myelogenous leukemia and least for acute lymphoblastic leukemia.

    • Cavazzana-Calvo M, Stephan JL, Sarnacki S, Chevret S, Fromont C, de Coene C, le Deist F, Guy-Grand D, Fischer A: Attenuation of graft-versus-host disease and graft rejection by ex-vivo immunotoxin elimination of alloreactive T cells in an H-2 haplotype disparate mouse combination. Blood 1994, 83:288-298. Murine model in which donor-anti-host activity is selectively depleted in vitro by treating host-activated donor T cells with a toxin-conjugated antibody to the CD25 activation marker. This maneuver successfully reduced GVHD in a haploidentical murine model.

The strategy of donor vaccination to increase the anti-malignant T-cell response is likely to be increasingly used. However, the limitation of the present approach is that it has not been confirmed that donor T cells responding to myeloma protein idiotype also exert an anti-myeloma effect.
