Retrospective review of 1010 consecutive transthoracic echocardiograms ordered to rule out cardiac source of emoblism found a highly probable cardiac source in less than 3%. This was no different from the prevalence in 325 echocardiograms ordered for other reasons during the same period.

JACC 1994, 23: 1598-1603. Transesophageal echocardiography identified a potential cardiac source in 45% of 145 consecutive stroke and TIA patients. In prospective follow-up those with abnormal findings had a higher rate of recurrent events.

Executive Committee for the Asymptomatic Carotid Atherosclerosis Study: Endarterectomy for asymptomatic carotid artery stenosis. JAMA 1995, 273:1421-1428. This long-awaited study found a 55% reduction in stroke risk with carotid endarterectomy compared with medical therapy alone in asymptomatic carotid stenosis. The absolute risk reduction was small, about 1% per year. The benefit was much less in women, primarily because of a higher surgical complication rate.
