Arch Intern Med 1994, 154: 1449-1457. The authors of five recent atrial fibrillation primary prevention trials pooled their data to produce a larger database. This analysis provides the best information on risk factors for stroke in atrial fibrillation. It also provides more powerful data on the effect of warfarin and aspirin.

N Engl J Med 1994, 331:1474-1479. This case-control study suggests that atheromotous disease of the aortic arch may be an important and underrecognized source of stroke. The clinical implications of these findings need to be examined in a prospective trial.

    • Endarterectomy for asymptomatic carotid artery stenosis
    • Executive Committee for the Asymptomatic Carotid Atherosclerosis Study: Endarterectomy for asymptomatic carotid artery stenosis. JAMA 1995, 273:1421-1428. This long-awaited study found a 55% reduction in stroke risk with carotid endarterectomy compared with medical therapy alone in asymptomatic carotid stenosis. The absolute risk reduction was small, about 1% per year. The benefit was much less in women, primarily because of a higher surgical complication rate.
