Two hundred and sixty previously healthy children aged 3-12 years with radiographically confirmed community-acquired pneumonia were evaluated and randomized to clarithromycin or erythromycin treatment. C. pneumoniae infection was identified by serology or nasopharyngeal culture in 28%, and M. pneumoniae infection was identified by serology or nasopharyngeal culture or PCR in 27% of patients. Clarithromycin and erythromycin were both > 90% clinically effective.

C. pneumoniae was identified as the etiology of acute respiratory infection by serological criteria for 3% (21/743) of adult outpatients studied and was identified in 3% of subjects with pneumonia, 5% of those with bronchitis and 5% of those with primary sinusitis. C. pneumoniae was detected by PCR in nasopharyngeal swab specimens from 15 of 20 serologically positive subjects tested by both methods.

Sera obtained concurrently was tested by MIF; one of the two C. pneumoniae positive subjects had an IgG titer of 1: 256 whereas the other had no detectable antibody. This study demonstrates that asymptomatic upper airway infection and carriage may occur in healthy persons.

Fifteen adult patients with acute exacerbation of chronic asthma were evaluated by acute and convalescent serology for C. pneumoniae, M. pneumoniae, Legionella pneumophila and common viral pathogens, and an indirect immunofluorescence test for C. pneumoniae on pharyngeal swab specimens. Seven of the 15 had serological evidence of acute C. pneumoniae infection, two of the seven had positive pharyngeal swab specimens for C. pneumoniae, and one of the 15 had serological evidence of acute M. pneumoniae infection.

J Infect Dis 1995, 172:585-588. C. pneumoniae was detected by PCR or ICC in 20 of 38 (53%) coronary atheroma specimens obtained during directional coronary atherectomy. Within the atheroma, the organism was localized to macrophages using cell identity markers. There was a trend towards an increased frequency of detection of the organism in specimens from patients treated for restenosis compared with patients with primary lesions (10/15 versus 10/23, P = 0.17).

C. pneumoniae was isolated from the coronary artery of an explanted heart removed prior to heart transplantation from a 56 year-old male with severe coronary artery disease. This result was replicated by the three study sites that received the sample. This is the first isolation of C. pneumoniae from a human atherosclerotic arterial specimen.
