Sydney SV, Mallory TH: Controlled perforation: a safe method of cement removal from the femoral canal. Clin Orthop 1990, 253:168-172. A technique of controlled femoral perforation during revision hip arthroplasty is detailed in this study. This technique was utilized in a series of 219 patients; 9 femoral fractures occurred postoperatively, but the authors indicate that only one fracture occurred at the site of a controlled perforation.

Access to the femoral cement during revision surgery was facilitated in this study by the use of a femoral window. The authors replaced the window and noted rapid healing with no perioperative fractures in 21 cases.

An extended trochanteric osteotomy that allows for wider exposure of the femoral canal is described. The results for 21 patients revealed union in 100% of cases, with no broken wires or trochanteric migration.

Review of the authors' laboratory and clinical experience with ultrasonically driven cement removal tools. No deleterious effects to bone where cement had been removed by ultrasound were demonstrated. Clinically, ultrasound technology helped avoid inadvertent and controlled femoral perforations during revision surgery.

McCallum JD, Hozack WJ: Recementing a femoral component into cement. Clin Orthop 1996, in press. In this study a femoral prosthesis was recemented into an existing stable cement mantle, that had been reshaped with ultrasonic tools, in 15 patients. Bone perforation and fracture were avoided and early follow-up suggested no adverse effect on the bone-cement interface was determined radiographically.
