Koretz reviews prospective randomized controlled trials in 3 categories: 1) evaluation of nutritional support (PN or enteral versus no support); 2) PN versus enteral; and 3) use of special nutrients, and states that controlled trials assessing PN efficacy have not demonstrated any significant favorable outcome in critically ill patients.

Randomised study of biochemical tolerance of intravenous glucose, amino acids, and lipid. Arch Dis Child 1995, 73:F8-F12. Low birth weight neonates 48 hours after birth were randomly assigned to either 10% dextrose; 10% dextrose with amino acids; or 10% dextrose, amino acids, and lipid (1 g/kg/d). A rapid decrease in plasma amino acids in group one was observed as compared with no differences in the amino acid profile between groups two and three. The amino acid solution used in the study (Vamin 9, Pharmacia, Milton Keynes, UK) resulted in higher levels of tyrosine and phenylalanine. The authors recommend an amino acid solution with reduced amounts of these amino acids. Vamin 9 is not supplemented with taurine, and low taurine levels were observed in all three groups. In this study, hypoglycemia occurred most frequently in group two and least often in group three. Plasma triglyceride and cholesterol concentrations were within normal limits for the three groups, indicating the capability to metabolize lipids with a 1 g/kg/d dose, although free fatty acids were not measured.

Low birth weight infants (n = 20) were randomly assigned to receive either 8 g/kg/d (5.5 mg/kg/min) or 12 g/kg/d (8.3 mg/kg/min) glucose. During this time, fat and protein intake were kept constant, at 1.8 g/kg/d and 1.5 g/kg/d, respectively. The authors found that during the 12 g/kg/d infusion, use and retention of glucose was greater. Retention during the 8 g/kg/d barely met use demands. Protein use was not significantly affected by the different levels of glucose intake, and moderate nitrogen retention was attained during both regimens.

The study's goals were to test the tolerance of glutamine in premature infants and evaluate potential benefits of glutamine-supplemented PN using such clinical indications as length of stay, days on PN, mechanical ventilation, and incidence of infections. Glutamine supplementation appears to be safe for premature infants. However, a larger, multi-institutional, randomized, prospective, and longer-term follow-up clinical trial is necessary to ascertain safety and benefits of glutamine supplementation.
