Glycohemoglobin levels of 157 adolescents with IDDM, aged 12 to 20 years, were evaluated, along with treatment adherence and psychosocial family data, through interviews and self-reports. A family-centered model for IDDM care is analyzed with a structural modeling statistical program. As expected, this approach to diabetes care appears to facilitate positive family functioning, reduces stress, and may promote improved metabolic control.

Grey M, Cameron ME, Lipman TH, Thurber FW: Psychosocial status of children with diabetes in the first two years after diagnosis. Diabetes Care 1995, 18:1330-1336. Eighty-nine patients with IDDM were compared with 53 children without the disease to determine the pattern of adjustment over time. The age range of the children was 8 to 14 years. Psychosocial status was assessed using several psychologic and behavioral testing instruments. Two years after the diagnosis, depression, dependency, and withdrawal were significantly more common in children with diabetes than in their peers without the disease. The 2 years following diagnosis are presented as a critical period for the acquisition of adaptation skills and a time for health care providers to be vigilant for the exacerbation of psychosocial concerns.

A fascinating study that stresses the importance of recognizing the prevalence of diabetes mismanagement among adolescent patients. A total of 144 adolescent IDDM patients, aged 11 to 19 years, completed a confidential questionnaire, and glycohemoglobin levels were measured. Questionnaires were also completed by a group of 55 parents who were asked to predict the responses of their children. Additionally, 56 pediatric endocrinologists were asked to estimate the frequency of mismanagement behavior among adolescents in their practice. Both parents and the physicians were found to underestimate the frequency with which teenagers engage in a variety of mismanagement behaviors. Teens were found to fabricate blood test results because of perceived pressure from their family and physicians to produce good results.

A longitudinal study documenting height, weight, skeletal age, and pubertal development in 46 children from initial diagnosis of IDDM until the attainment of final height. The study provides more reliable data than previous studies on this subject utilizing a cross-sectional design. Surprisingly, unlike other studies, no correlation was found between growth and metabolic control.

An interesting study that postulates that accelerated transport of glucose to the brain is a mechanism of unawareness of hypoglycemia in patients with IDDM. Brain glucose uptake was measured in 24 IDDM patients at normal and hypoglycemic plasma levels and compared with uptake in 15 normal subjects at these experimental parameters. There was no significant difference in brain glucose uptake in patients with the lowest glycosylated hemoglobin values. The responses of plasma epinephrine and pancreatic polypeptide levels as well as symptoms of hypoglycemia were lowest in the group with the lowest glycosylated hemoglobin values.

J Am Acad Child Adolesc Psychiatry 1995, 34:291-296. Forty-three girls and 35 boys, aged 8 to 13 years at IDDM onset, were followed up to determine the prevalence of DSM-III eating disorders and symptoms of maladaptive dietary or insulin management and their psychiatric and biomedical correlates. By a mean age of 21 years, 3.8% had DSM-III eating disorders and 11.4% had "eating problems," exhibiting severe dietary indiscretion and repeated insulin omission. Youth with "eating problems" were nine times more likely to have had a psychiatric disorder than the rest of the patients and had a significantly higher rate of medical noncompliance. Patients who exhibit serious dietary indiscretion and repeated insulin omission appear to have other psychiatric problems as well.

A total of 462 adolescent high school students were given a set of scales to determine their concerns about eating and perceptions of family and peer intimacy, social support, self-esteem, depression, and exercise. Fifty percent of the sample claimed to have concerns about undereating or overeating. Both groups had low self-esteem scores. The group concerned about overeating also scored higher on the depression scale.
