Prophylactic versus therapeutic platelet transfusion practices in hematology and/or oncology patients

Transfusion 1995, 35:498-502. This survey of hospital members of the American Association of Blood Banks collected data by questionnaire for the year 1992. Only 40.4% of the questionnaires were returned, and it is possible that a selection bias may have occurred. Nevertheless the 786 institutions that reported data represent a substantial proportion of US hospitals where patients with hematologic and oncologic disease are treated.

Gynecol Oncol 1995, 59:191-193. in this study 100 episodes of chemotherapy-induced severe thrombocytopenia (platelet count < 20,000/μL) were evaluated in 46 patients.The threshold for prophylactic transfusions was set at 5000/μL during a part of the study but did not result in significant increases in bleeding complications. This study provides further evidence that in stable patients the threshold for prophylactic transfusion can safely be set at counts lower than 20,000/μL.

This retrospective study of 213 pediatric patients is the largest report so far of alloimmunization in children with acute leukemia. Alloimmunization occurred in nine of the 31 patients with acute myeloid leukemia (31%), but only three of 133 patients with acute lymphoblastic leukemia (2.3%). The rate of 31% for children with acute myeloid leukemia approximates that reported for adults in other studies.

6 leukocytes/product of single-donor, ABO-compatible platelets did not appreciably affect alloimmunization rates among previously pregnant women with hematologic malignancies. Twenty of 46 patients developed anti-HLA antibodies and were equally distributed in both groups. The alloimmunization rate of 44% is similar to that generally reported for patients with acute myeloid leukemia, unselected for pregnancy or previous transfusions.

Transfusion 1995, 35:931-935. Exclusively single-donor apheresis platelets were used in this study. All units as well as all erythrocyte units were gamma irradiated and filtered. The study included patients with leukemia, lymphoma, and breast cancer, in almost equal proportions. Alloimmunization rates were low, as measured by lymphocytotoxic antibody. The discussion in this paper includes a summary of the alloimmunization rates found in other studies.

Transfusion 1995, 35:645-652. One hundred twenty-eight consecutive patients were studied. Alloimmunization occurred in 24%. As in adults, the lymphocytotoxic antibody assay predicted refractoriness to random donor transfusions but good response to HLA-matched units. There was no difference in alloimmunization rates between patients receiv-ing filtered products and those receiving unfiltered products.

Blood 1995, 86:3598-3603. A prospective randomized trial comparing filtered blood products to cytomegalovirus-seronegative products in the prevention of transfusion-transmitted cytomegalovirus infection after bone marrow transplantation. This large trial, involving 521 patients of whom 502 were evaluable, demonstrated a low risk of cytomegalovirus infection in both arms. Filtration may be effective for the prevention of transfusion-transmitted cytomegalovirus infection.
