J Clin Invest 1995, 95:2004-2011. Toxin B has little enterotoxic effect in animals. However, this study indicates that toxin B may be even more damaging than toxin A when applied to the human colon. This has important implications for human vaccine development and for other novel treatments, such as passive immunotherapy, aimed at binding to and neutralizing C. difficile toxins.

This study identifies the intracellular enzymatic action of toxin A which is identical to that described for toxin B (see reference 18). Toxin A catalyses the glucosylation of certain Rho proteins (RhoA, Rac1 and Cdc42Hs). Toxin A, like toxin A, uses UDP-glucose as its substrate and glucosylates a threonine residue in position 37 of the RhoA molecule.

This agrees with the observation that, in animal models, toxin A rather than toxin B is the prime enterotoxin. Oral passive immunization via breast milk was also demonstrated. Immunized female hamsters secreted antitoxin in their milk which prevented C. difficile colitis in suckling infant hamsters.
