Tomezsko JL, et al.: Energy expenditure and genotype of children with CF. Pediatr Res 1994, 35:451-460. Total (TEE) and resting (REE) energy expenditure were determined in 25 well prepubertal CF patients with mild-moderate lung disease, and 25 healthy controls. The CF group had a 9% increase in REE with no evidence of significant effect of genotype or pulmonary function. TEE was increased by 12% in the entire CF group and by 23% in the DF508 CF subgroup compared with controls. This is at variance with previous reports of normal TEE in CF. Further studies of detailed physical activity indices in CF subjects in various genotypic groups are indicated.

Management of EFA deficiency in CF has been to supplement with predominantly omega-6 fatty acids (particularly linoleic acid). However, as a precursor of the eicosanoid leukotriene LBT4, it has been hypothesized that use of omega-6 fatty acids may augment the levels of inflammatory mediators. In attempting to attenuate such a response, dietary supplementation with omega-3 fatty acids has been used preferentially. The result was a significant reduction in serum LBT4. A therapeutic role for the anti-inflammatory effects of omega-3 fatty acids in CF management is as yet unclear.

The reliability of commonly used predictive equations for estimating energy expenditure in infants both in health and in disease was assessed. Compared with measured values, predicted values for CF patients were underestimated. It suggested re-examination of currently used formulae.

J Am Diet Assoc 1995, 95(1):30-33. Thirty-one preadolescent CF patients had measurement made of resting energy expenditure. Harris Benedict equations and CF Consensus Committee equations underestimated energy expenditures by 13% and 8% respectively.

Assessment of 21 CF patients using an expanded scoring system based on nutritional and pulmonary function indices. This multiparameter score was felt to provide an effective and sensitive index of disease severity and is advocated in measuring effect of intervention and disease progression.

Calorie intake, eating styles and parental perceptions of eating behaviour in 32 CF children aged 2-5 years were compared with 29 healthy peers. The observation confirmed previous findings that CF children tend to eat as much as or more than their peers but are not meeting CF recommendations. There were also behavioural differences in eating and parental perception of children's eating that may influence sub-optimal nutrient intakes.
