This CASS registry study of 9557 patients demonstrates the importance of complete revascularisation, the use of the IMA, and cessation of smoking in reducing postoperative angina and myocardial infarction.

Seventy-four consectutive patients with ejection fraction less than 30% under-went CABG with low a mortality and a good 7-year survival (83.8%), but only 26% showed improvement in heart failure.

J Am Coll Cardiol 1995, 25:1000-1009. An important 15-year follow-up of 12,452 men and 2366 women in the CASS registry. Women had a higher hospital mortality from CABG, but long-term survival and surgical benefit were similar in both groups. There did not appear to be a sex bias in the use of angiography and surgery.

Relatively infrequent use of the IMA may underestimate long-term benefits from surgery in this study

This study of 121 octogenarians showed that although mortality and morbidity are higher in this age group, long term-functional outcome is relatively good in the survivors, with only 6.6% needing nursing home care.

This editorial describes the results of the Post-CABG Trial in which an aggressive lipid-lowering policy led to a 25% risk reduction in cardiovascular events, including repeat surgery, after CABG. This, as well as the Cholesterol and Recurrent Events (CARE) trial and the Scandinavian Simvastatin Survival Study (4S) raise important public and preventative health questions, including the question of the role of targeted versus population-based approaches.
