This article describes the use of serum thyroglobulin levels, as measured by a radioimmunoassay kit adapted for use with filter paper specimens, for defining the etiology of congenital hypothyroidism. The results suggest that thyroglobulin levels provide a reliable marker for the presence or absence of the thyroid gland, concentrations being elevated in children with ectopic thyroid glands and undetectable in athyrotic patients. However, there is a considerable overlap with values observed in normal full-term infants.

The use of thyroglobulin measurements as an adjunct to TSH levels is recommended in this paper, which demonstrates that, although thyroglobulin levels are dependent on TSH, they may be more useful in assessing compliance in children with an ectopic thyroid gland. This is because thyroglobulin has a longer half-life than TSH. However, a case cannot be made for using thyroglobulin levels instead of TSH levels in children with congenital hypothyroidism.

This paper reports that, in a group of 45 infants with congenital hypothyroidism, early treatment (median age at commencement of treatment, 14 days) at a median initial dose of levothyroxine of 11.6 μg/kg/d, which is higher than that used in previous studies by the same group, resulted in similar mean developmental quotients at 18 months in children with severe and moderate congenital hypothyroidism, suggesting that the developmental gap between these two categories of patients can be closed by early high-dose levothyroxine treatment.

In a study of 23 neonates with congenital hypothyroidism treated with thyroxine, mean dose of 6.6 μg/kg/d, the IQ scores were comparable to those of a control group, and hence, the authors conclude that there is no need to treat congenital hypothyroidism with higher doses of thyroxine and that TSH normalization should not be a primary objective of treatment.

This seminal paper describes the threshold effect of the pretreatment thyroxine level on IQ. The data suggest that the severity of prenatal hypothyroxinemia will determine the degree of brain damage and hence IQ.

Pediatr Res 1995, 37:736-740. This paper described the attention deficit and delayed nerve conduction velocity observed in infants with congenital hypothyroidism diagnosed in the prescreening era. Additionally, 47% of neonates detected by screening manifested an abnormality in at least one neurophysiologic test.
