During an epidemic of multidrug-resistant typhoid in southern Viel Nam an open paired randomized study compared ofloxacin 10mg/kg a day for 5 days with 15mg/kg daily for 3 days in 438 patients with uncomplicated typhoid fever, of these 286 were children, 228 were blood culture-positive and 91% of the S. typhi were multidrug resistant. There was only one treatment failure in a patient who took only one dose of ofloxacin. Of these patients 70% were followed up at 1 month and there were no carriers identified on the basis of culture of a single stool sample.

In this study 63 culture-positive adults responded equally well to 400 mg fleroxacin once a day for 3 or 5 days; 84% of the S. typhi were multidrug resistant. Mean fever clearance was 89 h in the 3-day group and 82 h in those treated for 5 days. The one possible relapse was in a patient treated for 5 days.

In this study short courses of either 2 or 3 days of ofloxacin (15mg/kg/day) were compared in 100 children with typhoid fever; 86% of isolates were multidrug resistant. Six of 53 children treated for 2 days were clinical failures compared with two of the 47 treated for 3 days. Fever clearance was about 100 h in both groups. One child had a suspected relapse and one carrier was identified.

In this study 326 Vietnamese children were followed up for 2 years after receiving either ciprofloxacin (97 mg/kg given over 7 days) or ofloxacin (45-50 mg/kg given over 3-5 days) for treatment during the control of an epidemic of multidrug-resistant typhoid fever. Growth velocity and weight for height were compared with an age-matched control group of 223 children from the same locality. There was no evidence of acute joint toxicity or joint symptoms attributable to the fluoroquinolones and there were no differences in growth or the expected weight or height measurements between the three groups of children over the 2-year period.

In this study 12 isolates of S. typhi from Vellore, India with reduced susceptibility to 4-quinolones (MIC of ciprofloxacin 0.25μg/ml) were found to have mutations in the 'quinolone resistance determining region' of gyrA. In nine isolates phenylalanine was substituted for serine at position 83 of gyrA, in one tyrosine was substituted for aspartate at position 87 and the final isolate had a combination of both mutations. These are similar to the mutations seen in other ciprofloxacin-resistant enterobactericeae.
