This study demonstrated that patients with unstable angina had a high incidence of coronary lesions associated with white platelet-rich thrombus, and patients with acute myocardial infarction had a high incidence of lesions associated with erythrocyte and fibrin-rich thrombi. These findings have implications for treatment strategies for patients with acute ischemic syndromes.

This suggests that the majority of patients with unstable angina have other mechanisms such as plaque inflammation responsible for the precipitation of their acute ischemic syndromes.

This careful analysis revealed that heparin in concentrations conventionally thought to be therapeutic (0.2-0.4 u/ml) has only 20% of the thrombin-neutralizing activity against clot-bound thrombin which it has against soluble thrombin. The direct thrombin inhibitor, hirugen, has equivalent inhibitory effects against clot-bound and soluble thrombin.

In this phase III trial of patients undergoing angioplasty for unstable or post-infarction angina, Hirulog therapy resulted in slightly lower rates of major ischemic complications and strikingly lower rates of major hemorrhage.
