Ophthalmic Surg and Lasers 1995, 26(suppl 5):414-419. This paper is a retrospective study in which the authors compare IOP reduction in phacoemulsification-trabeculectomy and ECCE-trabeculectomy eyes The mean IOP reduction was significantly greater in the phacoemulsification-trabeculectomy group; there was no significant difference between the groups in visual acuity improvement or glaucoma medication reduction. The basis for the preoperative assignment to one or the other surgical group is not described.

This prospective study quantifies early and late complications in a series of 49 eyes (36 patients) undergoing phacoemulsification-trabeculectomy surgery with a polyHema IOL followed up for 24 to 46 months (mean, 31.5 months). Fibrin effusion was a common, early complication (61%). Iridocapsular synechia (57%) and posterior capsular opacification (59%) were common, late complications. Filtering bleb failure was found in only 8%. Combined small incision cataract surgery and trabeculectomy is concluded to be a successful approach for long-term visual rehabilitation and glaucoma control.

J Cataract Refract Surg 1995, 21:528-532. This paper describes a phacoemulsification-trabeculectomy surgical technique using a central radial incision to open the roof of the scleral tunnel to facilitate a punch sclerectomy of the floor of the tunnel. The incision can then be closed with a single horizontal suture, allowing postoperative titration of aqueous outflow by laser suture lysis if indicated. In a preliminary series of 24 consecutive eyes, a mean IOP reduction of 5.0 ±2.4 mm Hg was found.

Munden PM, Alward WLM: Combined phacoemulsification, posterior chamber intraocular lens implantation, and trabeculectomy with mitomycin C. Am J Ophthalmol 1995, 119:20-29. This paper is a retrospective study of 21 consecutive patients having phacoemulsification-trabeculectomy surgery with adjunctive mitomycin C. Intraocular pressure fell from a preoperative level of 20.8 ±5.0 mm Hg to 15.1 ±3.1 mm Hg. No patient had decreased V/A postoperatively, 81% had BCVA 20/40 or better. Fifteen patients (71%) were using no glaucoma medications at last follow-up. Follow-up was for a minimum of 6 months. Careful use of adjunctive mitomycin C in phacoemulsification trabeculectomy surgery was concluded to be safe and effective.

This study is primarily of interest for the duration of experience with TAB in combination with diverse techniques of cataract extraction. The recommendations were made that TAB is not the appropriate procedure if the target IOP is in the low teens, patients may be incapable of tolerating postoperative IOP fluctuations in normal tension glaucoma, or in the presence of severe optic nerve damage.

J Cataract Refract Surg 1995, 21:653-660. This is a prospective, randomized, controlled study comparing IOP in phacoemulsification patients with and without TAB Entry criteria do not clearly differentiate ocular hypertensives from POAG. The two groups differed in percentage taking a beta-blocker (70% TAB, 44% control) and in initial IOP, the TAB group was higher, P - 0.07 (not statistically significant). These may both have favored the small but statistically significant difference in IOP reduction observed (a greater fall of IOP in the TAB group). A reduction of 3.8 mm Hg in the mean IOP of the control group can be attributed to the effects of cataract extraction alone.

This paper details the use of endocyclophotocoagulation as an adjunctive treatment to cataract extraction in a small series of 10 patients with uncontrolled open-angle glaucoma. Selection criteria were not specified and uncontrolled glaucoma was not defined. Complications were minimal, vision and IOP improved, and postoperative course was typical of cataract rather than glaucoma surgery or cyclo-ablative procedure patients.
