This study is the first systematic estimate of the concordance rate among twins with ankylosing spondylitis. The 50% concordance among identical twins (compared with 15% to 20% among dizygotic twins) indicates that disease expression of ankylosing spondylitis is largely genetically controlled.

    • Yamaguchi A, Tsuchiya N, Mitsui H, Shiota M, Ogawa A, Tokunaga K, Yoshinoya S, Juji T, Ito K: Association of HLA-B39 with HLA-B27-negative ankylosing spondylitis and pauciarticular juvenile rheumatoid arthritis in Japanese patients. Arthritis Rheum 1995, 38:1672-1677. HLA-B27 and HLA-B39 share amino acid residues critical to formation of the B pocket of the peptide-binding groove. The data showing that HLA-B39 is found in B27-negative ankylosing spondylitis patients (Gladman et al., Arthritis Rheum 1995, 38:845-850) and data from this study showing that HLA-B39 affects the progression of psoriatic arthritis may indicate that the presence of these amino acids is important to the pathogenesis of spondyloarthropathy.

Comparison with HLA-27-positive healthy control subjects in a limiting dilution analysis and determination of the specificity of synovial fluid T cell clones. Arthritis Rheum 1995, 38:1277-1282. The authors found quantitatively fewer T cells reactive with Klebsiella in the peripheral blood of ankylosing spondylitis patients compared with controls. Like most good science this interesting study raises more questions than it answers. What is the mechanism for the reduced number of and-Klebsiella T cells in the blood of ankylosing spondylitis patients?

    • Scofield RH, Kurien B, Gross T, WL Warren, Harley JB: HLA-B27 binding of peptide from its own sequence and similar peptides from bacteria: implications for spondyloarthropathies. Lancet 1995, 345:1542-1544. A peptide from B27 is bound by B27 as are a series of peptides from bacteria that mimic the B27-derived peptide. The B27-derived and B27-bound peptide, as well as the bacterial peptides binding and mimicking B27, are the basis for a new theory of disease that combines the molecular mimicry and arthritogenic peptide hypotheses.

    • The promiscuous B27 hypothesis
    • Davenport MP: The promiscuous B27 hypothesis. Lancet 1995, 346:500-501. Perhaps B27 binding itself is not critical to pathogenesis of spondyloarthropathy, but instead B27 peptide(s) bound and presented by HLA class II (and mimicked by bacteria ?) are critical. Such a peptide would need to be bound by many, if not all, HLA class II complexes in order to explain the lack of association of spondyloarthropathy with HLA class II.
